I acknowledge that participation in athletic training with Game Winner Athletics Facility involves inherent risks of physical injury. On behalf of the athlete, I voluntarily assume all such risks and release Game Winner Athletics, its owners, coaches, and staff from any liability for injury, loss, or damage arising from participation, except in cases of gross negligence. I confirm the athlete is physically able to participate and that I am authorized to sign as parent/guardian.
